Don't strain yourself, that smaller print reads: Sweet, tender "real" crab and cheddar omelet.
Crystal spotted this one in Albany NY. She asked the waitress for clarification, but I still wouldn't probably order that one...

They got the apostrophe in crab lovers' right, so I'd have to think they have a reason for the "real" quotation marks. Unfortunately this weakens their case for good food.
While it's safe to love eating crabs with delight
I'm not so sure about those being crabs' lovers and delighted
So make sure you have crabs as food in sight
And not become the crabs' lovers and be slighted
